I currently have a few users that have multiple sessions active and they are on different app servers. I will have to do some asking to find out what tools they were using. The only tool that was reported with this problem was with Tests and Quizes.

I don't think you need a balancer member "not available" for that, just a configuration that results in requests being forwarded to the app servers more or less at random.
With a four hour session timeout people could easily have a session open on both app servers and things might work out for them - or not - simply depending on coincidence and what tools they're using and how those tools have been written.
I don't remember how the "online"-tool in the admin workspace looked in Sakai 2.9, but at least in current versions it can display the list of sessions/ users on each app server.
This might be worth a look, because should the problem be that peoples'
sessions don't "stick" to one of the app servers, then I would expect several users to have open sessions on both app servers.
